The presence of left atrial posterior wall fibrillation despite restoration of sinus rhythm after posterior box ablation

چکیده
Introduction Catheter ablation plays an important role in the management of persistent atrial fibrillation (AF), though recurrences of AF following ablation in patients with this condition remains high. There is general agreement that catheter ablation for AF should include pulmonary vein antral isolation (PVAI). The location and extent of ablation that should be performed beyond PVAI for the management of persistent AF remains uncertain. Although controversial, the targeting of the posterior left atrial wall for ablation beyond isolation of the pulmonary veins (PVs) has been promoted by some operators. We present a case demonstrating the potential importance in some patients of targeting the posterior wall for ablation.

Outcome of stand-alone thoracoscopic epicardial left atrial posterior box isolation with bipolar radiofrequency energy for longstanding persistent atrial fibrillation
INTRODUCTION Catheter ablation of longstanding (> 1 year) persistent atrial fibrillation (AF) is associated with poor outcome. This might be due to remodelling and fibrosis formation, mainly located in the posterior left atrial (LA) wall. Therefore, we adopted a thoracoscopic epicardial box isolation of the posterior left atrium using bipolar RF energy with intraoperative testing of conduction ...
متن کاملA new method to evaluate linear block at the left atrial roof: is it reliable without pacing?
OBJECTIVE The present study aimed to evaluate a new method for validation of complete linear block at the left atrial (LA) roof. BACKGROUND Linear lesions at the LA roof have been reported to improve the success rate of catheter ablation of atrial fibrillation (AF). METHODS Complete linear block at the LA roof was evaluated in 31 patients after complete isolation of pulmonary vein antrum (P...

A 58-year-old man with hypertension and diabetes with recurrent persistent symptomatic atrial fibrillation resistant to sotalol-facilitated cardioversions was referred for ablation. Catheter-based radiofrequency ablation for persistent atrial fibrillation was performed, which consisted of isolation of all 4 pulmonic veins; linear ablations along the left atrial roof; an ablation line from the l...
متن کاملCardiac rhythm and atrial transport function after surgical ablation of atrial fibrillation using cryoenergy: predictors and effectiveness of the procedure.
AIM The aim of this prospective study was to assess the presence of sinus rhythm and atrial transport function after surgical ablation of atrial fibrillation using cryoenergy, and to evaluate predictors of the success of the procedure. METHODS Between January 2005 and September 2006, 100 consecutive patients underwent left atrial cryoablation as a concomitant surgical procedure (46 patients w...
متن کاملCardiac Remodeling After Atrial Fibrillation Ablation.
Radiofrequency catheter ablation procedures are considered a reasonable option for patients with symptomatic, drug refractory atrial fibrillation (AF). Ablation procedures have been reported to effectively restore sinus rhythm and provide long-term relief of symptoms. Both electrical and structural remodeling occurs with AF. A reversal of the electrical remodeling develops within 1 week after r...
